Hi I just run into this on a fresh Karmic install, #77 cured the problem, in spite I applied 38. I found that the problem was on the pulse audio mixer, I could unmute and crank up all volumes at the alsa mixer without anything happening. Bug 456013 may be the expression of the same symptoms.
