Attached is the icon image that I see in my PulseaAudio volume control.
There is practically no difference in what it looks like selected or
unselected so it's very hard to tell when mute is on or off.

I don't see any linked chain icons in gnome-volume-control, but it has
left-right balance control and rear-front fade control (in 4.0 hardware
profile). I think it would be best, if there were separate locks from
rear to front and from front left to front right and from rear left to
rear right.

It does use a muted speaker icon now, but the icon doesn't change. If
you press the button in, it's muted, and if you unpress it, it's
playing, but the icon is a muted speaker no matter what.

This currently shows a "lock" icon for ganging the faders, and the icon
is far away from the faders, making its function not obvious. I would
expect a "lock" icon to lock the device and prevent other programs from
changing it.
Instead, you should show a "chain link" icon, and show lines connectin
