I have one reproducible test case (at least in my case, using Jaunty):
if I have a DVD with a windows game (The Sims 2) in the drive, and I
click on the "Places" menu the first time after login into Gnome, the
icons are not shown until the drive spun up and the icon of the game is
being loaded. Thi
I have that problem on ubuntu jaunty. In my laptopn (semprom, 512mb ram)
I login into gnome and press the menu button. It takes about 3-4 seconds
until I can see it. The new users (or old users :) can think Ubuntu is
slow (really it is slow the first time I open the menu)

I can confirm that the loading is still taking place when the icons are
needed, but the loading times seem to have somewhat improved in karmic.
I did however test this when there was little other disk activity, I
suspect that the loading lag would be longer when other disk activity is
taking place.
Ok downloading the karmic ISO now I will try it on a liveCD

Try a fresh reboot, and then navigate the menu. Once they're loaded,
it doesn't happen again.

I agree, this looks very bad and is very confusing.
The icons should be pre-loaded.
