Hello.
I have a 3g modem zte mf100, and am experiencing exactly the same issues as
above.
When I plug the modem, I have to kill modem-manager, in order that my
connection in network manager becomes available (or reboot computer).

Ubuntu 9.10 (karmic)
kernel: 2.6.31-17-generic

Have resolved this issue for me. My 3G modem connects initially without
killing modemmanager
The device no longer connects initially either. Need to killall modem-
manager first, then it will connect.
