Having the same issue. Custom cursor is displayed ok in some apps (e.g. Firefox and GNOME Terminal), but in other apps (GNOME Commander, Rhytmbox). in configuration dialogs and on desktop the cursor switches to the default one. This only happens while running compiz, i.e. when Appearance->Visual
