removing it entirely since the odds of me using the modem on this
machine are less than zero.

This is suddenly affecting me too. Didn't exist on a pre-final Karmic
install. Started maybe 2 weeks ago? My pci id and "fuser ..." output
are the same as Paolo4500's above. The laptop is a Sager NP2092 which

working, second boot.. not working, pulse applet show as dummy audio

You could try installing linux-backports-modules-alsa-karmic and reboot and see
if that fixes the problem.

Same problem for me. However, i get sound output every time, if I
1. Start Ubuntu
2. plug in a pair of headphones
3. Rightclick on the volume-icon top-right and choose Sound settings
4. Go to the Output-tab
5. Choose Analog Headphones
6. Choose Analog Output again
