It occurs to me only when shutting down my machine while firefox is
still open. It appeared on Kubuntu 13.10, 14.04 and 15.04 (all versions
of Kubuntu that I've been using in the last few years).

I have a new install of Karmic Kubuntu (not an upgrade) and Firefox package
version is 3.5.7+nobinonly-0ubuntu0.9.10.1.
This is a laptop with a LUKS-encrypted HDD, so I have enabled the auto-login
feature of KDM.
Every time I boot-up, KDE restarts firefox (if it was up when I shut down) and

This is the standard restore message from Firefox allowing you to choose which
pages you want to restrore. This is not a bug.

This is most likely an issue with Firefox not being capable of properly
restoring the pages after resuming. I'm setting the package back to
'firefox-3.5' and am subscribing the MozillaTeam, they know more about
Firefox and could help with determining the cause of this problem.

My apologies...I didn't realize that you were referring to the session
restore window. There should be a fix for apport on Karmic soon. Would
you be able to try to produce a crash report?

Actually, this is not a Firefox issue. That's the standard Session
Restore message. That should happen on Shutdown so that your tabs are
restored. It should not happen on Hibernation though. I would check
into why your hibernation is not working properly.
