FWIW, I just discovered today that it also happens in chrome so it seems my
issue is with the flash plugin ubuntu, not with firefox.

Same issue here, for the last few weeks but mine hasn't gone away. I
have to restart firefox at least once a day or just use an alternate
browser for flash content. OGV works fine. Unlike previous issues, I
don't need to restart pulseaudio -- just firefox. And FWIW, I can use
flash in other

I use Ubuntu 9.10 Karmic Koala and Firefox 3.6 from the Ubuntuzilla
repository.
I'll also like to say I havent chanced upon this issue in quite a while
since this report was made. Perhaps the update fixed it or some other
modification took place.
