Public bug reported:

Binary package hint: pulseaudio

I use an A2DP headset as my only sound output (I blacklisted
snd_hda_intel in /etc/modprobe.d; this is probably why I saw errors
during ubuntu-bug just now of /dev/snd and /proc/asound missing). The
volume control in the system tray generally works.

If I don't play any audio for a while (a minute or so?) the headset will
disconnect, and Pulseaudio will immediately reconnect, silently except
for a brief flicker in the system tray. So far so good. The problem is
that the effective volume at this point is 100% and I get deafened. The
tray volume icon still shows the old volume, and a slight nudge of the
mouse wheel over it is sufficient to reapply the volume setting.

Expected behaviour is for the current system volume to take effect on
the A2DP device (as it does when adjusted) immediately on
(re-)connecting to the device.
