I bought a dvi-hdmi adapter and I am now able to activate the second
display on the radeon hd 3450. So, it looks like VGA and DVI video out
are working, HDMI video is not.

Yeah, outputs and modes are set up by the kernel drm now, so this is a
linux KMS issue. I'll refile for you.

It seems my bug report may be similiar to

I also have a Radeon HD3450 connected via HDMI. However, that reporter
seems to experience intermittent issues and in my case the monitor never
gets output.
