This problem was resolved for me after installing the libhal1 and
hal both version 0.5.14-0ubuntu6 update (and then reboot).
Even more previously added icons to the indicator applet which
dissapeared after upgrade from 9.04 to 10.04 now also suddenly appear
again.

Hi Max,
Please check that you have the indicator sound library (libsoundmenu.so)
in /usr/lib/indicators/3/. If so please kill the service and readd the
applet to the panel (as described above). Then please attach the log
file for the applet to this bug. The log file can be found in ~/.cache
I'm running on 2.6.32-22-generic #33-Ubuntu SMP and pkgs being up-to-
date till may 8th 2010 and (still) have the issue of not having the
sound icon on the I-A (v 0.3.6). This started after my updrade where I
went from 9.04 via 9.10 to 10.04 on one day.
10.04 indicator-sound pkg version:
-b :)

Yes, I-A in the Panel. Its About dialog says Indicator Applet 0.3.6 .
I don't understand -b.
I am stabbing in the dark by interpreting Name Request and attaching
the dbus logs. Hope this helps.

Hi Chad,
Sorry the b was a typo hence the -b in the second message.
Okay so at start up or after reloading the indicator-applet is there any
speaker on the panel ? Which indicators are loaded ?

No speaker exists in the I-A panel or in the old Notification Area. In
the Indicator Applet, there are 1) display settings, 2) Bluetooth.

Bugs,
I'm a beta tester for U 10.04, so it is possible my upgrade schedule has
screwed up something in testing.
I worked around the problem by removing the Indicator Applet from the
Panel, killing indicator-sound-service, and adding the Indicator Applet
back to the panel.
Please, future testers, try

Hi Chad,
The service needs to have an indicator in order to continue running. Has
indicator-applet been added to the panel ?
