Yep I got it with the program called GTK Radiant (it's a map editor):

To reproduce: Install lucid-amd64-server.iso, install virtualbox-ose-qt,
run virtualbox, click new.

I had a similar problem. It turns out QGtkStyle tries to discover the
theme with GConf, found that out by strace'ing qtconfig-qt4.
So I installed gconf-editor to pull in GConf, then it worked. I agree
it's a bug in QGtkStyle - at the very least, it should spit out that it
can't load GConf.
