I guess the menu bar problem is a problem for all Java apps, or
perhaps Eclipse is special. In any case you can get the menus back by
running 'unset UBUNTU_MENUPROXY' in a terminal and then launching
eclipse from that terminal.
The icon mess up is prolly also related to Java apps somehow, that
the java issue with appmenu is bug #659931

It happens only with SWT based apps (like eclipse and tuxguitar).
I've made a little app to test this on Swing and the menu is rendered in
Java (so, it does not integrate to Unity). See attached screenshot.

Ah... and with Swing the icon is correctly displayed too.
