To reset the entries in the menu please open terminal and type
gsettings reset com.canonical.indicators.sound blacklisted-media-players
I think this is a more an update bug as opposed to one which will reoccur.

Yeah Jono the fix for this in the bug mentioned above by Robert, it
seems to be some race condition after an update as there is no reason
for this behaviour in the code base.

There is bug #701435 describing the same issue, (I had it), and it was
set as Fix Released, and I haven't seen Banshee duplicated since then,
but yesterday, with all updates I had the same bug in the Messaging
menu, so it might not be only sound-menu specific:
I had two instances of running
