What about adding something like a glow effect when scrolling the
document without using the scrollbar directly to underline where the bar
is?

Any change to the overlay scrollbars is a good change, as far as I'm
concerned. With my 1920x1200 monitors, I'd much rather save time than
free up a few pixels. The overlay scrollbars have been buggy and
sometimes unusable for me, anyway. I definitely agree with the OP, that
the worst part can b

Another possibility is, when the cursor approaches the scrollbar edge,
to show a traditional scrollbar with the scrolling thumb in a
(semitransparent?) scroll channel. Clicking in the channel would page
up/down the document, per traditional scrollbars.

I agree with the OP, this is quite a serious usability issue in the otherwise
pretty
neat overlay scrollbars. In practice, the longer the document, the more
important
scrolling, the less reachable the scrollbar.
A very simple solution - for which I can't really see any drawbacks - would be
