utils/defaults to /etc/pm/config.d/ and then edited that file, like
described in the header of the file.

I had the same problem with several Linux distributions (*Ubuntu,
Fedora, Debian, Suse, Mint, and many more, yes I've installed them all
in the last months) but now I solved this problem in Arch Linux by
editing:
/usr/lib/pm-utils/defaults:

Yes, this happens every time. I can not determine if the disk spins
down; the HD light certainly goes off. Adding max_cpus=1 makes no
difference.
How can I get some more debug output?
The end of pm-suspend.log says
/usr/lib/pm-utils/sleep.d/99video hibernate hibernate: success.

Is this issue consistently reproducible? Do the hard disks spin down
prior to the system hanging? Does it still happen when you boot with
max_cpus=1 on the Linux kernel command line?
