Hmm, then it's probably some kind of race condition, or that the USB
stack creates devices in the wrong order somehow, but it's probably hard
to debug without having the hardware at hand...

Interesting problem, never heard of this type before. I suspect the
hardware somehow isn't ready when Pulseaudio probes it. Could you do
provide a PulseAudio log: http://wiki.ubuntu.com/PulseAudio/Log and plug
it in while the log is running (and confirm the problem)? Thanks!
