In addition, Libreoffice apps do not show up in the task switcher, which
is really annoying. However, they still show up in expose mode for
windows (Ctrl+w).

After resetting all values to default in dconf-editor desktop unity
launcher I rebooted. The problem still remains for me.
To recreate the bug:
1. Remove all Libre Office icons from unity launcher.
2. Open Dash and search for LibreOffice.
3. Launch the 'LibreOffice' entry rather than one of
I can confirm this. See screenshot.
It doesn't happen all the time. Once in a while there will be a blank document
icon in the launcher.
