I believe the problem is not that different icons are being shown for
different filetypes, but that multiple instances of the icon for Olular
are being displayed in the Unity Launcher because BAMF is reporting
these as different applications (not as different instances of the same
application).

Okular does not use different icons when you open a pdf and when you
open a dvi, what made you think that? Did you even try it before
declaring it was weird for doing that?
And not, it's not starting different instances for pdf and dvi, it's
just starting a new instance of the same binary for eve
