using terminal to perform both operation (update and upgrade).

I experienced the same thing after an upgrade from 11.10 to 12.04. Clicking on
"install updates" would cause update-manager to appear to install updates
without actually doing anything.
I ended up accidentally resolving the issue by selecting "Mark all upgrades" in
synaptic.
