There's no specific steps that will reproduce it reliably, but it
happens randomly ALL THE TIME.
When a new window is opened, some times it is given focus and sometimes
is not.
This is a pretty well known bug that dates back to Gnome, and is the
same in Unity, so please don't let it expire.
