I saw the same problem, and suspect it is related to gnome-tweak-tool,
because the problem is gone after I remove that package.

To clarify, I'm observing this intermittently when I lock my screen. If
I run Chrome full-screen, I don't have the problem. I just re-locked my
screen after putting Chrome into full-screen and the bug did not present
itself.

I had this issue on 12.04, and I believe on 11.10 before that...but have
made it go away by switching between Desktop Cube to Desktop Wall and
back to Cube in ccsm (which requires disabling and re-enabling unity, so
not entirely recommended I guess!)
In the process I noted that I had Gnome Compati
I've never had this issue. Does it occur with all fullscreen apps (try
checking it with Firefox, Chrome(ium), Totem, VLC, etc)? Does it occur
always? And, most importantly, which Ubuntu version are you on? And
which Unity version?
