As I had just installed the 11.10 using the same USB stick with success,
instead of getting another stick I just reloaded the most recent 12.04
installer image made a startup disk and what do you know: the problem(s)
are gone now - not sure what has changed, but this bug can be closed.

the USB drive (sdb) not the internal one

I did install Ubuntu 11.10 back on the same HW without a problem and I
am using it daily without any problems. The hardrive is IBM SDD drive.
12.04 also had hard time downloading the 11.10 image without crc errors
and in the end I used other computer to do it.
I can boot to 12.04 from USB stick
Thank you for taking the time to report this bug and helping to make
Ubuntu better. Reviewing your log files attached to this bug report it
seems that there is a problem with the disk to which you are trying to
install Ubuntu. I'd recommend performing a back up of your existing
