I have a similar issue; when running a pdf file (using evince) from
nautilus, focus remains on nautilus, while the pdf file covers the
screen.

Both unity unity-2d can also show similar with the 'Downloads' dialog
window in FF Open Containing Folder, though in the case of unity-2d
positioning of the FF window is a factor. Most likely to occur when FF
is in the upper left corner
