I doubt PA has anything to do with it, but gstreamer might. In any case,
does the upstream link mean this is no longer incomplete?

I am using amarok - kde player and can reproduce the bug when changing
from mp3 to ogg.
So I think the problem comes from pulseaudio or deeper.

Could somebody having the issue report the bug upstream:

I experience the same bug on a fresh 12.04 i386 install. My music
collection consists of MP3 and OGG Files. As described the bug manifests
itself, wenn an MP3-file is followed by an OGG file.

I can confirm that this did not happen on 11.10 with the same music
collection. I have ogg, mp3, and wma.

I can also confirm this problem. I have also a mixture off mp3 and ogg files.
With Ubuntu 11.10 it works fine.

kde - player amarok has the same problem.
I also have this problem. I can confirm Otus' findings on what
transitions cause the hang.
If I enable crossfade, instead of hanging indefinitely on transition,
there's about a 5-10 second hang, then Rhythmbox skips partway into the
next song and carries on as normal.

I can reproduce 100% of the time and crossfade doesn't help. Playback
sometimes continues after a while (3-4 minutes in the most recent case I
saw) and the song jumps to a position close to where it would have been
if it had been playing from the beginning.
The following transitions cause a pause:
Yep, I believe this is a duplicate of bug 921071 (can reproduce by going
from mp3 -> ogg) but the suggested solution of enabling crossfade
doesn't seem to work for me.
