I've just had this problem, and solved it. Check the perms on your /var/lib/ldap directory and files to ensure that the 'openldap' user can write to it. I had mine left as 'root' from having just done a 'slapadd' :) See also: http://www.openldap.org/lists/openldap-software/200508/msg00044.html
