Just to be clear, I used a USB to PS2 adapter to collect the above
information. I don't want this to be a red herring and be targeted as
the cause of the issue. Using Ubuntu 12.04 on this motherboard, the PS/2
ports are unusable and the only way to pass any input to the OS is via
USB.

Same here. PS/2 keyboard stops working randomly. After rebooting, the keyboard
is functional again for sometime but it eventually quits working. This has
started upon upgrading to 12.04. Currently I am on:
3.2.0-25-generic #40-Ubuntu SMP Wed May 23 20:30:51 UTC 2012 x86_64 x86_64
x86_64 GNU/Linu

Upgraded to 12.04 from 10.04, using USB thumbdrive. Tried re-installing
twice, same result. My PS/2 keyboard doesn't get detected. It works well
with 10.04.
Btw, My system has two drives and ubuntu is installed on the second
drive, in multi boot mode.
Running the self test, will post the results

i have same issue at my desktop pc, it denies a wired ps2 keyboard after grub
and i can't use on screen keyboard too. so i can't gather debug info
but there is no problem on 12.04 live version at same pc
