Thanks for your report. I've been able to reproduce the issue and by
digging a bit deeper I found two relevant bugs in Debian [1,2]. Also
relevant is the changelog of the Debian exim4 package [3].
In my understanding this is what happened:
* exim4 4.92 broke the compatibility with sa-exim [4].
Suffering from this as well, making kernel upgrades on serveral machines quite
hard (need to boot with selinux=0).
I can reproduce the failure with the steps above on 4.15.18-38, while Debian
buster/sid with 4.19.28-2 doesn't show the problem.

Hi Mark, from some Googling it looks like XFCE itself has some issues
with menu bars. Could you try running the following to see if the menus
re-appear (it may need a reboot too, not sure):
xfwm4
xfwm4 --replace
If that doesn't help, try (again, might need a reboot):
rm .config/xfce

Tested with 5.0.0-1ubuntu4~ppa1 from PPA now all three vhost hotplug
