login credentials, the screen flickers and then it takes me to the GUI
login prompt.

eject has to be issued twice, as it will fail the first time, succeeding
the second time.
$ sudo eject /dev/sr0
eject: unable to eject, last error: Inappropriate ioctl for device
$ sudo eject /dev/sr0
It should simply eject. I don't know what the first eject does different
I located bug #578139 in the upstream at gnome.org that appears to also
be this issue. I reported on it the findings here and gave them this bug
number just incase.

I think I am hitting the same problem; interestingly on Xubuntu 14.04 ... and a
plantronics blackwire 420 headset.
Any time I use it to make a call (and start using its volume keys) ...

AFAIK, there's no way for ofono to know that credit has run out.
ofono is responsible for attaching to the GPRS network and ensuring that
the correct ConnectionManager DBus properties are exposed ( 'Powered'
and 'Attached' ). Likewise ofono exposes one or more GPRS contexts

This needs fixing in precise too. I just ran into that very problem with
current upstream LXC and a precise privileged container.
I'll upload a straight cherry-pick of the mounted-dev.conf job as we
have it nowadays (I confirmed that fixes it here).
