I've found many times this behavior with Ubuntu-Gnome 14.04 during some
package upgrades with update-manager. In this case, no "failed" message.
Only in some case, changing between [Enter] key and click [Ok] button
did the trick.

The same happened to me when I upgraded from Ubuntu Gnome 15.04 to 15.10
few days ago. I tried to kill the lock screen but could not find how. I
did a restart to fix it.

I will try and take a look at upgrades something in the next couple of days.
For now I have just uploaded a plymouth fix, that hopefully wipes out the
"check disc", "crypt password" and restart bugs. I will re-spin the
images once it migrates to -release pocket (about an ~hour)
