When using the volume up/down media buttons, I don't hear any difference but in
the Volume control pannel it's visible that the channels get unlocked from each
other (button changes to unlocked)

I have the same problem as explained here but the situation in bug #126333
(which this one is tagged as duplicate of) doesn't seem to be the same problem.
I don't remember if it is happening since I upgrade
Removing Pulseaudio worked for me too.
Follow next explanation and reboot.
