Not really a bug in Sound Menu. Last time I checked Clementine
implements it's own indicator applet.

In Quadrapassel set the Starting level sufficiently high (i.e. level 20)
so that the bricks will fall fast and end the game quickly. Exit
Quadrapassel and X will crash upon exiting Quadrapassel and throw a
bunch of errors in Xorg.0.log.

Here is a backtrace.
I hope this is sufficient but there is still a ?? beside libglx.so. I
have installed the debug symbols for xserver-xorg-core-dbg, libgl1-mesa-
dri-dbg, xserver-xorg-video-ati-dbg and xorg-video-radeon-dbg plus a
bunch of others.

I have run the back trace again but this time with packages from the
xorg-edgers ppa. I was using this ppa previously when I discovered the
crash. I downgraded everything to the Lucid packages to test and make
the bug report.
I updated the packages again to xorg-edgers to try the back trace and
I have been downloading the updates regularly. This crash hasn't
occurred since updating.
Is it possible that this would cause a kernel panic? I have been getting
those once in a while now unfortunately no error shows up in the kernel,
syslog or xorg logs so I don't know if it's related.

I found solution.
Set your application font-size to something that's not using a fractional part.
For example if you have set your main
application font-size to 7.5 set it to either 7.0 or 8.0

I also have this problem on my HP nc8000.
It works when I do echo 0 /proc/acpi/video/*/DOS. I tried the above
and disabled the hotkeys service and this also fixes it.
I have checked the lid state in /proc/acpi/button/lid/C153/state.
When the DOS value is 1 the state does not change. It is
