Sometimes orca stops completely in my karmic 64 bits box.
With some sighted help I found two things:
1: No sd_espeak running, only sd_dummy.
2: speech-dispatcher was using too much CPU.
As soon I killed speech-dispatcher orca returned again.
This is happening with some frequency.
