I've suffered similar issues to this with the MX4. The USB connection
degraded in reliability for me over time, and I ended up having to get a
replacement MX4, which so-far has been good.

I still cannot get any "menu" or any other visible functionality in
recovery mode, but adb functionality issue was solved in an interesting
way, here's steps I went through with help of Dave and others in
#ubuntu-touch
1. flash the mentioned recovery.img with: fastboot flash recovery
