I was able to reproduce this issue again for the 3rd time on a clean
install. I believe that anybody can reproduce this with the following
steps:
STEP 1: Do a clean Ubuntu Server 10.04 installation. When I installed, I
selected "OpenSSH server" as the only additional installation package.
STEP 2:
I have finally physically got to where the machine is and rebooted it
after making the changes to /etc/pam.d/common-auth. This has solved the
issues I originally posted here. It has not solved the portmap issues I
mentioned above which indeed seems to be a completely separate issue.

In the long run, the solution is to have freshclam use a helper
application. The helper application would run unconfined but would only
run stuff that is in those directories, and the main freshclam binary
wouldn't be able to write to those directories.

The default apparmor profile for the freshclam binary doesn't contain
rules for scripts added to the /etc/clamav/onupdateexecute.d directory
as we can't predict what those scripts will be doing.
You can fix this is one of three ways:

Yes, as stated in my original comment, it is reproducible (myself and
Dustin Kirkland), with variants of "while true; do sudo multipath -F;
sudo multipath -v4 ; done".
All that is required is a device that multipath will create a
/dev/mapper entry for (use_friendly_names or not) and remove with -F
So that means it fails about 10% of the time. Could you attach your
cronjob so that we reproduce with the closest environment ?
