Okay, I found a great workaround for me:
1. I made sure I removed light-locker, gnome-screensaver, xscreensaver,
kscreensaver, etc..
2. installed xscreensaver
3. reboot
Standby/resume and screen unlocking now works perfectly. While xmodmap
is still broken, I don't care about that.

Thanks Timo - I only had light-locker installed, so I did this:
1. Uninstall light-locker
2. Install xcreensaver
3. Reboot
Agreed, it seems to be working fine now, so might be something related
either to light-locker or how it interacts with xfsettingsd.

configuration which is similar to the setup with two monitors. The bug does not
occur: one cursor per conlsole copy. However, gpm had to be installed and run

Hmm, besides "We don’t recommend users to update the BIOS if their
system is already running normally" (which I have read before
upgrading), the official site also says "Please only flash newer
versions of BIOS in the future". Also, version of BIOS that I had isn't
available for download. Now what?
Besides fancier icons in BIOS, the only visible change is that wired
network card isn't working after wake-up. I tried a couple of times,
same thing (ifconfig lists only lo after wake-up). Also, now, trying
logout and login after that locks up the system (at least I can
Ctrl+Alt+Del from terminal).
I am not sure - I don't have x64 installed - and I am reluctant to do - I don't
have a spare partition to use.
It would take a lot of effort to completely recreate my settings on an x64 -
unless I have misunderstood what you asked.

Prior to some installations, more feedback I can give for now is that the
problem also persists with
- gpm version 1.20.4-6.1
- xorg version 1:7.7+1ubuntu8.1
- two physical monitors attached through DVI
- the arrow, non-functional cursor is displayed on exactly one of them
- all
It is the same situation with v3.16-utopic. Closing the lid with ext
monitor connected causes windows to move to ext monitor but does not
suspend.
I have discovered another symptom that may possibly be related. With
any of the kernels and with the external monitor not connected, closing
