I am experiencing this problem for the first time, and have recently upgraded
to Ubuntu 11.04 64-bit - possible regression?
When I connect an external monitor to my Lenovo ThinkPad X200, Intel 4500MHD
graphics, the mouse is invisible on the second display (extended desktop mode).
Also, I get

Compiz has a plugin that can prevent windows from getting offscreen. It does
not restrict the cursor though, and I don't know about desktop icon placement.
Activate the Put window plugin, go to Misc. Options and enable Avoid
Offscreen.
It would be nice if this functionality could be implemented

I've the same as songohan82, except I do not face this problem every day (lucky
me!) and that for my part it doesn't seem to be related to Firefox at all.
I'd like to add that I'm a frequent user of suspend when I travel
to/from work, just in case this is a factor.
