It does seem to only affect me when I specify 1600x1200.

second screen in dualhead setup doesn't refresh properly
Interestingly, gnome-screenshot doesn't see the display that doesn't
refresh properly. Whereas before the screenshot would be super-wide and
cover both displays, now it's regular 4:3 and covers only the display
that refreshes properly. Maybe that somehow is helpful to those that
know the code?

I'm not sure how I would disable EXA. I searched a bit and found
http://web.archiveorange.com/archive/v/Ky751tZ4E5diWGWdfeKP but they say
there are no alternatives to EXA on R600 or newer (I have R600).

Started happening for me after upgrading to Natty and disabling Unity.
The workaround mentioned in bug #378482 (tiling backgrounds) didn't
help, although switching to metacity does (but that has its own
issues...)
