I quite often get this problem with 12.04.
Intel(R) Core(TM) i7-3770S CPU
my Xorg.0.log says this when it does not work (see attachment for when
it does work):
[12.269] vesa: Ignoring device with a bound kernel driver
[12.269] (WW) Falling back to old probe method for vesa
[12.516]
Now I cannot reproduce it either, I also get fullscreen. I don't know
what I changed to get that effect.
I wasn't really trying to get fullscreen though, but that's better than
a misaligned window.

When running xterm in Unity, the window is horizontally misaligned, it
is placed too far to the right, the rightmost part of the window is off
the screen. See attached screenshot.
