I didn't find the crash report under /var/crash, but re-produced the issue and
save the crashed kernel log as the attachment.

I didn't duplicate the issue with an external 1920x1080 monitor.
I watch the issue with an external 1280x1024 monitor.

Anthony, I have tried the patch, it seems still not resolving the issue.
I seems to me that ACPI event not coming from the kernel, please verify.
And for the video switch hotkey, please attach an extended monitor,
using Fn+F8(may different from platforms) to switch amon extended
mode/internal

1. configure haswell platform with 2 monitors
2. start X or lightdm will crash Ubuntu kernel, see the crash errors attached.
