I'm seeing in dmesg logs:

[15869.816022] [drm] GPU crash dump saved to /sys/class/drm/card0/error
[15869.816023] [drm] GPU hangs can indicate a bug anywhere in the entire gfx 
stack, including userspace.
[15869.816024] [drm] Please file a _new_ bug report on bugs.freedesktop.org 
against DRI -> DRM/Intel
[15869.816026] [drm] drm/i915 developers can then reassign to the right 
component if it's not a kernel issue.
[15869.816027] [drm] The gpu crash dump is required to analyze gpu hangs, so 
please always attach it.

This started sometime in the last couple weeks on trusty.

It is moderately annoying in that apport kicks in and walks me through a
collection process and then (I think) *it* dies, so I was unable to
actually report the bug.

Instead, I reported this bug with:
  ubuntu-bug xserver-xorg-video-intel

One thing that might be interesting is that /sys/class/drm/card0/error
is an empty file. (and so is /sys/kernel/debug/dri/0/i915_error_state)
