As this is an output detection issue when running with kernel
modesetting, since the kernel is now in charge of setting up and
managing the video outputs, I'm bumping this over to the kernel team.
One other question - did this use to work and has regressed, or has it
never worked as you expect in
Removing the HDMI cable still fails to cause a reconfiguration. The
second screen stays active until you use Monitor preferences which then
detects the change. 'Apply' then switches the second screen off.
Still no HDMI available unless the cable is attached when the bios
initialises.
