I've found a solution: edit file /etc/pulse/default.pa
reach line load-module module-udev-detect
and add this to the end ignore_dB=1
I don't know if this change has side effects, but it works

When I modify the volume with the multimedia keys, the PCM volume is
not modified.

Same for me. I have a dell inspiron 6400 with HDA Intel audio card.
When I open alsamixer and I lower my volume to 21%, the master volume is
muted (=0).
If I lower my volume less than 21%, the pcm volume is lowered.
In Jaunty, the pcm volume isn't lowered when I lower the general
volume.
It is
Same here using an Audigy card.
