I think that I might have spotted the problem. This is a fresh install of
ACS 4.5 and Xserver 6.5 and I noticed that the vlans are not being created
in xenserver. The vlans are configured in ACS but they are not being pushed
to the xenservers. I am using openvswitch and not linux bridge on xenserv

Is the compute offering you are using for your VMs set to use local storage?
You will need to create new compute offerings if not and launch your VMs with
those offerings.

Are you having multiple zones? On latest 4.5 branch, I registered an ISO
with a fake URL and it failed with the download failed 404 error. After
that, I was able to delete the ISO using UI by selecting suitable zones.
