Thanks for sharing the libvirt / qemu versions. May be, some compatibility
issue with these versions. You can try with other cloud images there.

I've tried with cloudimg-amd64.img (as raw template) in hypervisor with libvirt
4.5.0 / qemu 1.5.3, on cloudstack 4.16.1, and it is booting fine.
Got the same issue with images cloudimg-amd64-disk-kvm.img,
cloudimg-ppc64el.img (as raw templates) in hypervisor with libvirt 4.5.0 / qem
