I see what you mean. I probably didn't look closely enough at your examples.
Rightly or wrongly however, I always have the en-dash with spaces fore and aft,
and never use the em-dash. To be honest, until relatively recently I just used
a hyphen anyway. I suppose that comes from having used a typ
En- dash/em-dash
All I need to do is type either double hyphen or triple hyphen to get the
appropriate dash. No need for colons etc. I have never changed any autocorrect
settings to achieve this. It is triggered by the second space, whether
immediately after the hyphen or after the next word.
I use the alternative find and replace add-on and can recommend it.

I've had similar issues with user defined labels. Make sure to check the
spacing between labels is correctly set. If it is fractionally out, you
will get this sort of cumulative increase in misalignment.

I think I've had similar problems in the past, which I have put down to
corruption of the file. I think I saved as a txt file and then back to odt.
Obviously that means reapplying all the styles in use, which may be an
issue with complex documents.
