Hi All,
Recently we noticed that Apache (2.0.59) on RHEL 3 as a forward proxy
stops responding for a few minutes and then recovers on its own.
We have MaxRequestsPerChild to 9000 and the occurrence is random
(fortunately over weeks)
In the error log we get the lines for the time period where Apache
stopped responding:

[Tue Mar 20 09:40:01 2007] [error] [client 81.207.47.250] request
failed: error reading the headers, referer:
http://by126w.bay126.mail.live.com/mail/mail.aspx


Looking at the source code, we find that this error is reported from
protocol.c - after calling ap_get_mime_headers_core(r, tmp_bb); method.
In this method we see that Apache is writing the error to r->notes
("error-notes") than the error log.

So we are clueless what made Apache to return this error message.

>From the mod_log_config module we see that we can log the same by using

 
 % {error-notes} N  

to the log format string.

Is it wise to add the "% {error-notes} N" to the log format? 
It is a production server with 15,000 users on it.

We would like to see what the reason for this error is in the error log.

Can any of you suggest us the direction to look in to?
