Not exactly, but something like this is possible - the script will run on every
INVITE transaction, but it will do a query only once at first run (once per
every process/child).
You run a perl script for example on every INVITE, inside the script you have
something like:

I was getting 500 Server Errors in the replies. The errorlog showed memory

Found the problem - it seams that the older version of openldap-devel
defines ldap_url_search() which overlaps with the the same function
defined in the ldap module.
Can you try to install a newer version of openldap-devel (2.1 or higher) ?

FIFO is just a macro :). A typical fifo name is /tmp/opensips_fifo for
example.
So, put something like this in the "fifo_name" modparam and also in
opensipsctlrc for OSIPS_FIFO
