Probably you don't trigger accounting for BYE request from the script (or not
to all BYE requests).
If you want also to account CANCELS there's a flag in the acc module to do it
(as far as I remember it doesn't enabled by default).
If accounting is triggered for BYE request - it produces ACC ST

You get an error in log, but 500 is not sent. As far as I remember, there was
no way to fix it more cleanly.
The situation can be easy reproduced if you "slow down" processing of INVITE
and then send CANCEL, for example with exec_dset("very_slow_scrip
between radius server and your application server.
