alternate selected, but CACHE_EVENT_OPEN_READ_FAILED

I have ATS (3.2.4) set up as a reverse proxy. During the request I see several alternates proposed, and the last is selected. However, I get a CACHE_EVENT_OPEN_READ_FAILED. Does this mean the object is not found in cache? How can this happen given the alternate has already been selected? I'm new
